Senior Day Services at Tabor Hills

Tabor Hills offers a warm, welcoming, home-like environment for participants to engage in large or small group activities or individual pursuits all under the watchful eye of trained staff. There is a room specifically designed for rest and relaxation with recliners, soft lighting, calm music and aromatherapy.


All participants have access to programs and services on campus. With a physician referral, physical, occupational, and speech therapy is available to participants in our state-of-the-art rehabilitation suite. Podiatry care (if in network) and a hair salon (for a reasonable charge) are also available. Senior Day participants may also attend performances and activities in our Healthcare and Supportive Living Facilities. 


The program is for seniors over the age of 55 who would benefit from the socialization, stimulation, supervision, safety and security offered by a senior day program, as well as caregivers who need to regain a sense of themselves and would benefit from respite from the duties of caregiving.


Senior Day Services is a structured program that is consistent and predictable, but flexible enough to accommodate the different interests of the group.

We provide a variety of activities addressing the seven dimensions of wellness:

 

  • Physical : utilizing movement to enhance one's well-being and improve range of motion and circulation, providing nutritious meals and snacks, monitoring of chronic health conditions, administering medications, recognizing early signs of illness
  • Emotional : encouraging the expression of one's feelings, offering security and the feeling of being loved, focusing on participant's abilities rather than their disabilities
  • Intellectual : stimulating creativity, engaging in problem solving, creating opportunities to learn new skills
  • Social : creating a sense of belonging and self-worth
  • Spiritual : creating meaning, purpose, reflection 
  • Environmental : appreciating the environment around us and our place in it
  • Occupational : making use of individual skills and talents to foster a sense of well-being and purpose

    Monday - Friday 7:30AM - 5:30PM

    Flexible hours for both partial and full-day attendance

  • Staff

    Our Program Director is a registered nurse (RN) with over 40 years' experience in the care of older adults. Supplementary staff include a certified nurses' aide (CNA), campus maintenance, housekeeping, and dietary services. 

  • Cost of Attendance

    - $75 for partial day attendance (5 hours or less per day)

    - $110 for full day attendance (5 hours or more per day) 

    - Private Pay

    - Long Term Care Insurance (check your policy regarding possible benefits)

    - Senior Day Services is not covered under Medicare

    - Community or state funding is not available

Getting Started

  • Visit the campus, take a tour and meet the staff!
  • Complete an Application (see button below). 
  • Determine a start date and the number of days participant will be attending (2 days/week is recommended for consistency and familiarity). Changes can be made as needed with reasonable notice.
  • Have health care practitioner complete Medical Evaluation and Authorization to Participate form and provide a medication list.
  • Complete Lifestyle and Functional Assessment.
  • Meet with Nurse/Program Coordinator for in-person assessment of participant (if not already done).
  • Complete and sign required participation forms. 
  • Provide center with copies of advance directives, insurance cards, and COVID vaccination records.
  • Plan for any additional services. Provide personal supplies such as body soap/shampoo if participant will receive bathing services. Make a standing appointment at the hair salon.
  • Be sure orders have been received for outpatient speech, occupational, and/or physical therapy.

First Day at Senior Day Services

  • Bring any medications the participant will need during attendance hours. Medications must be in recent, labeled prescription bottles.  
  • Bring a sweater, change of clothing and any personal items needed (Ex. incontinence products, hygiene wipes). These will be stored during the day in either a locker or storage bin.
  • At this time, participant will receive a TB test in his/her forearm (a 2nd test will be administered again in 2 weeks to confirm negative results, unless a QuantiFERON lab test was performed prior).

Additional Information

  • There are male/female bathrooms within the center equipped with handrails, raised toilet seat and space to accommodate those with assistive devices. 
  • Shower facilities are available for as-needed situations and for regular grooming if this is a difficult task at home (for an additional fee).

Communication

  • A service plan is created within the first two weeks that identifies the goals of attendance and strategies to obtain them. This service plan will then be reviewed annually or more often as necessary.
  • A written update will be provided to caregivers and health care practitioners regularly informing them of the participant's level of involvement in the program, appetite, toileting and hygiene, socialization efforts and any behavioral concerns.
  • Program staff are available at the time of drop-off and pick-up or by phone/email during operating hours. Messages may be left on voicemail after hours. Families are encouraged to ask questions, share concerns, and offer feedback at any time. Communication is key to the participant's success and the caregiver's ability to trust.
