  • What type of care is provided at Tabor Hills Healthcare Facility?

    Tabor Hills Healthcare facility is a long-term nursing and rehabilitation facility offering care in the following areas:

    • Short-term/subacute rehabilitation
    • Long-term care
    • Alzheimer and dementia care
    • Palliative care
    • Hospice care
    • Respite care
    • Medically complex care: IV, ostomy care, wound care, wound vac, diabetes I & II management, life vest, and more

  • What are visiting hours at Tabor Hills Healthcare Facility?

    It is our philosophy that visitation by family and friends remain a critically important part of the resident’s rehabilitation and daily lives. As such, visitation is permitted 24 hours per day for all guests of the resident, provided the visits are not negatively impacting the resident or any potential roommates.

    We do ask that visitation is deferred if you are experiencing any symptoms consistent with COVID or are within 10 days of a positive COVID-19 test.  We may also ask visitors to defer non-urgent visits if the visitor or the resident has had a recent known exposure. 

  • What services are available to residents and their families?

    • Podiatry, dental, ophthalmology, cardiology, pulmonology, nephrology, infectious disease consultations, and extensive wound care/vascular services are available within the facility

  • Will Medicare pay for a subacute rehabilitation stay?

    Medicare requires certain criteria be met for an eligible beneficiary to receive skilled nursing benefits.  The beneficiary must have an inpatient hospital 3 midnight stay, medical necessity must be certified by the hospital attending physician, and the skilled care must be related to the condition for which the beneficiary was hospitalized.  


    Medicare DOES NOT pay for custodial or intermediate care in a nursing home, even if it is a Medicare certified facility. Medicare’s nursing home coverage is limited to medically necessary skilled nursing and rehabilitation care following a hospital stay.

  • What should I expect upon admission?

    Upon arrival to Tabor Hills, you will be greeted by a nurse and certified nursing assistant for medical assessment and review of amenities throughout the building.  Depending on your time of admission, an admissions representative will complete admissions forms with you and/or your family. Your first full day, the Tabor Hills rehabilitation team will work to evaluate you, both medically and physically, and develop a plan of functioning to safely return home.  Your Tabor Hills physician will meet with you to complete a history and physical within 72 hours of your admission.

  • What should residents bring for their subacute rehabilitation stay?

    Please bring Medicare and insurance cards, as well and any advanced directives and power of attorney. Please provide information regarding whether you have received the flu and/or pneumonia vaccines.

    Bring enough clothing for one week.  

    • Comfortable clothing that is easy to put on and take off as desired.
    • One pair of tennis shoes/soft sole shoes
    • Underwear and socks
    • Pajamas and robe (gowns are provided)

    Please do not bring medication from home, since this will be provided through the facility’s contracted provider.  Personal hygiene products are provided, but you may bring from home if desired.  Bring dentures/partials, hearing aids, and glasses, but bring cases for safe keeping. Cell phone/tablet/laptop (free wi-fi available), but must be stored when not in use.  If you choose to activate your room phone with direct dial number, there is a charge of $1/day.


  • What is Tabor Hills long-term care (skilled care) admission process?

    Thank you for considering Tabor Hills for long-term care.  Please see link below to the long-term application form.  Once the application is completed and returned with copies of financial statements (see application), a member of the Admissions Department will contact you to discuss your application in more detail.

  • How do I know if my loved one needs skilled care or supportive living?

    Supportive living is appropriate for individuals unable to live on their own anymore, but don't require daily care.  This level of care provides meals, light housekeeping, laundry, assistance with bathing, and 24-hour CNA staff for emergency needs.


    Skilled nursing (long-term care) caters to those individuals whose conditions have declined to the point where they need daily physical and clinical attention beyond what an assisted living facility would offer.

  • Is Tabor Hills licensed in Illinois and certified by the Centers for Medicare and Medicaid? Why is that important?

    Tabor Hills is licensed by the Illinois Department of Public Health and certified by the Centers for Medicare and Medicaid Services (CMS).  This is important because it assures that Tabor Hills is a high-quality provider of care.

  • Is transportation available if I have a physician appointment during my stay?

    Transportation utilizing Tabor Hills vehicle for scheduled group transportation for special outings, church services, and activities is provided at no charge.


    Transportation cost provided by Tabor Hills for Sub-Acute Rehabilitation residents to physician appointments is billed to Medicare.


    Long-term residents or those under the respite program may utilize Tabor Hills transportation for personal/outside physician visits for a nominal fee. 


  • Can my pet come to visit while I am at Tabor Hills?

    Pets are welcome with a vaccination record provided prior to the first visit.  Pets are limited to cats and dogs, since other animals are known to be inhabited with potential dangerous bacteria.


    Short leashes are required.
