Outpatient Therapy
Tabor Hills Therapy Services provides physical, occupational, and speech therapy on an outpatient basis, when subacute/short-term rehabilitation is not appropriate or upon completion of your inpatient rehabilitation, when therapy is still prescribed.
Outpatient therapy is covered under Medicare Part B, when prescribed by patient’s physician.

• Back pain, neck pain, shoulder pain• Arthritis• Joint pain• Joint replacement• Balance retraining• Weakness• CVA/stroke• Parkinson’s disease care
Tabor Hills Healthcare Facility, Inc.-owns and operates the skilled nursing facility. Bohemian Home for the Aged is the parent corporation of Tabor Hills Supportive Living Community, LLC (its sole managing member) and Tabor Hills Healthcare Facility, Inc. (as its sole corporate member).
