CMS Five-Star Rated Facility

Tabor Hills has achieved and maintained an overall five-star healthcare facility rating by the Centers for Medicare & Medicaid Services for over five consecutive years. Only the top 10 percent of nursing facilities in the country achieve this high rating, making the distinction a coveted accomplishment. The CMS Five-Star Quality Rating System was created by CMS to provide unbiased information to help in making decisions about what nursing facility best meets you or your family member’s needs. The overall rating is based on measurements from health inspections, staffing and quality measures.

This rating system grew out of the Omnibus Reconciliation Act of 1987 and quality improvement campaigns, such as the Advancing Excellence in America’s Nursing Homes, which is a coalition of consumers, healthcare providers and nursing home professionals.
