We encourage our residents to enjoy their independence while at the same time offering them the necessary tools and support to be successful. 
The Supportive Living Community at Tabor Hills offers 3 different apartment styles and beautifully designed community space inside and outside for resident enjoyment. Our seniors are active, social, and fun. 
Tabor Hills Supportive Living is not just a place to live, it is a Home.

All Inclusive Apartment Features

  • Light housekeeping and laundering of bed linens and redressing of bed(s)
  • Utilities and maintenance
  • Two emergency alarms per unit│ Personal Pendants are available for a one-time purchase (alarms are monitored 24 hours a day).
  • Light housekeeping and laundry are included in rent. Laundry includes weekly bed linens and remaking of bed
  • Optional cable packages and wireless internet service are available for a nominal fee. (Phone is not included)
  • Medication reminders, including set-up are an important part of care and are included at no additional charge.

 

Dining Services

 

  • Three delicious home-cooked meals and a continental breakfast are available daily.
  • Snacks and beverages are available 24 hours a day. 
  • Special menus are created for holidays and events.
  • To maintain high resident satisfaction, monthly food forums are scheduled with the chef to incorporate Resident input on upcoming menus and special events. 
  • Residents say that the quality of the food served and our attention to detail is excellent.

  • Many great choices for an active and fun filled day at Tabor Hills, enjoyed with wonderful friends you have yet to meet to share the fun! 
  • Based upon resident preferences and interests, scheduled fun and entertaining activities are planned by a full time Activities Director, including:

  • Visits from Pet Therapy, Miniature Therapy Horses, and Animals from the Brookfield Zoo!

Medicaid Licensed through the Illinois Department of Healthcare and Family Services

 

  • Illinois developed the Supportive Living Program as an alternative to nursing home care for low-income seniors with physical disabilities under Medicaid. Supportive Living is like Assisted Living but there are differences due to the guidelines set forth by the Department of Healthcare and Family Services for the Medicaid program.
  • Tabor Hills Supportive Living Community accepts residents on Medicaid or to assist those living in our community whose assets spend down. For more specific information on the Medicaid program please direct your questions to the staff in the community at 630 -778-6677 x 1227.
  • In addition you are welcome to visit: https://www.illinois.gov/hfs/medicalprograms/slf/Pages/default.aspx for specific State of Illinois guidelines to the program.


Non-Discrimination Statement


  • No individual participating in the Supportive Living Program (SLP) shall be discriminated against because of race, color, religious belief, political affiliation, sex, national origin, or disability.
  • No person shall be improperly excluded from or denied participation in the SLP based solely on the diagnosis of mental illness if the State’s designated screeners find the individual otherwise eligible and appropriate for services within the Program. Following completion of this screening, the SLP Providers shall individually assess each eligible applicant in order to determine whether the SLP Provider can meet the person’s needs. The SLP Providers remain subject to all applicable State and Federal law, including the Americans with Disabilities Act and the Fair Housing Act.
