For our long-term residents, Tabor Hills utilizes person centered care which maintains abilities of the resident. This includes addressing specific needs of each resident, creating unique care plans to incorporate input from both residents and family to understand the resident’s needs and assure those needs are being addressed per their care plan.
The healthcare facility is designed to create a sense of neighborhoods, much like the ones from which our residents lived. Each "neighborhood” has its own dining and activity rooms which are short walks from the resident’s room. Tabor Hills is dedicated to helping seniors live in a community where quality of life, dignity and self-determination is the primary focus.
Tabor Hills Healthcare Facility, Inc.-owns and operates the skilled nursing facility. Bohemian Home for the Aged is the parent corporation of Tabor Hills Supportive Living Community, LLC (its sole managing member) and Tabor Hills Healthcare Facility, Inc. (as its sole corporate member).
